POSITION SUMMARY:
Program Specialist wanted to provide education and support to help individuals to quit smoking. Provides direct tobacco cessation services across CommuniCare and all partner organizations. Engages with all program participants and provides a comprehensive tobacco assessment and treatment plan. Provides tobacco cessation services in group and individual formats. Supports policy and practice change related to tobacco across CommuniCare and all partner organizations.

RESPONSIBILITIES:

• Provide individual and group counseling, education and support for tobacco cessation.
• Provide community education and outreach on smoking cessation related activities.
• Coordinate with Psychiatrist and/or PCP on the use of CCI tobacco cessation pharmacotherapy and/or nicotine replacement therapy.

LOCATION: New Haven

HOURS:
Monday – Friday 9:00am to 5:00pm

MINIMUM REQUIREMENTS:
Bachelor’s Degree required. Master’s degree preferred.

Experience in health and human services required.

Experience providing tobacco cessation services preferred.

Must be proficient with PCs and Microsoft Office Word & Outlook

Must have and maintain a valid CT driver’s license.

Also must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.

May be required to use own vehicle on company business and to transport clients.
