POSITION SUMMARY:
Responsible for engagement with families referred by the Department of Children and Families. Identifies needs of family to maintain in the community. Develops of a Plan of Care to address needs identified in program referral. Responsible for developing a rapport with all family participants, facilitating referrals to needed services, and working with the Parent Navigator to meet the needs of the families.
RESPONSIBILITIES:
• Conduct family needs assessments and develop Plan of Care that identifies family needs.
• Provide case management services to children and families.
• Coordinate family team meetings.
• Collaborate with other team members to ensure linkage to community resources and services.
LOCATION: Branford
HOURS: Monday – Friday 9:00am to 5:00pm
MINIMUM REQUIREMENTS:
• Bachelor’s degree required.
• Ability to work a flexible schedule.
• Sensitivity to diverse cultures and language competencies.
• Knowledge of behavioral health issues for children and families and of home-based services preferred.
• Good communication and documentation skills.
• Must be proficient with PCs and Microsoft Office Word & Outlook. Ability to prioritize and organize schedules/demands.
• Must have and maintain a valid CT driver’s license. Also must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
