• A new voluntary program for families who have received a Family Assessment Response from the Department of Children and Families. Referrals are made by DCF only.
• The Community Support for families program is designed to work with families in their homes and communities to identify resources and supports with the goal of reducing parental stress and increasing parental resiliency.
• Families that are referred and choose to participate in the program will work with a Family Support Specialist and at times, a Parent Navigator.
• The Family Support Specialist and Parent Navigator will work with the family to identify a team of people that the family would like to participate in their family team meetings.
• Family team meetings are coordinated and facilitated by the Family Support Specialist to identify the family’s strengths, needs, resources and strategies that will be used to help the family achieve their goals.
• Information from the Family team meeting is used to create a Plan of Care that outlines the family’s goals, identifies who will help the family and how the goals will be achieved.
• Community Support for Families program uses and endorses a Parent driven, family focused practice that incorporates natural supports and resources to meet their family’s needs and goals.
